Description of  how longstanding fragmentation of care in local large care home contributing to increasing reliance on non elective and prolonged admissions to secondary care,  has been addressed by multifactorial approach- namely introduction of an outreach Trust frailty liaison matron, improved MDT working , stakeholder collaboration and training, and shared IT. Case examples from liaison matron and GP trainee  will illustrate effectiveness

Prior to the pandemic, geriatrician input was face to face and on a smaller scale. After the first wave, as a result of enhanced funding, a more structured approach was implemented to deliver remote multidisciplinary care into all care homes in Tower Hamlets.

I have been supporting a local 90 bed nursing home as a partner at Manor Field Surgery in Maltby. In one and a half sessions a week I review the residents of the care home and support multidisciplinary working, comprehensive geriatric assessment, advance care planning and admission avoidance. Avoiding uncessary hospital attendances by frail older people is fundamental in ensuring wellbeing!
