Enhanced health for care homes: Hammersmith and Fulham; cases for frailty liaison matron and MDT intereventions.

Description of  how longstanding fragmentation of care in local large care home contributing to increasing reliance on non elective and prolonged admissions to secondary care,  has been addressed by multifactorial approach- namely introduction of an outreach Trust frailty liaison matron, improved MDT working , stakeholder collaboration and training, and shared IT. Case examples from liaison matron and GP trainee  will illustrate effectiveness
