HealthScores.ai by zPaper is a connected health app that brings a member's healthcare data from multiple provider portals and their wearables together into a single Health Profile, a secure record they own in their own Google Drive. We never take custody of it. The profile rolls up into a 0 to 1000 Health Score built from two parts: a baseline reflecting what a member's health history has already set, and a larger portion that moves as a member completes recommended actions. Members choose who joins their Care Circle and what health data each person sees, and share through QR codes built on Smart Health Links and FHIR. No clipboard required.
