Community Children's Nurse

The CCN team office is based on the Bristol site of the University Hospital Bristol and Weston Trust but are predominantly community working completing a wide variety of clinical tasks for Children and Young People (CYP) aged 0-18yrs, in the Bristol, South Gloucestershire and North Somerset (BNSSG) areas. The team's working hours are currently between 8am-6pm 7 days a week, 365 days a year. Must have full UK driving licence and car available for use daily. The CCN team enable swift discharge, admission avoidance, ambulatory care and support of the CYP of the BNSSG area. We also ensure efficient patient flow from Bristol Royal Hospital for Children (BRHC) and surrounding children's inpatient facilities working closely with BRHC emergency department and inpatient wards/teams.
