Community Mental Health Nurse

We work as an integrated health and social care team with great links to the community integrated neighbourhood teams to promote individual’s independence as they move towards their recovery and discharge from secondary specialist services. As a post holder you will undertake the planning and delivery of direct patient care with minimal supervision, ensuring continuity of practice by working within a multi-disciplinary and multi-agency environment. Managing a caseload and the provision of mental health care services to people with mental health care need. You will be responsible for accurate mental health clustering of all patients on admission, discharge and at review. Also, to ensure all mental health clustering information is passed onto other Trust teams or services on transfer of patients. To ensure accuracy and quality of mental health clustering activity within the team.
