Senior Mental Health Practitioner - OPMH (Bideford)

We are looking for a Senior Mental Health Practitioner to join the North Devon Older Peoples Mental Health team based at Bideford hospital and covering the North Devon area. The team includes, RMN's, OT, Consultant, psychologist, support workers and works with people who have organic or functional mental health problems. This post will be primarily working with clients experiencing organic mental health problems. You will be a core member of the community mental health team providing assessments and delivering intervention in line with clearly identified clinical pathways. You will have a positive approach to delivering care and team working promoting recovery, independence and well-being. You will have the opportunity to contribute to improvements in care delivery and will be encouraged to propose and participate in tests of change to improve care. Currently the North Devon team is participating in a Dementia Advisor Support Expansion pilot project which has independent evaluation taking place from Plymouth University. The North Devon team operates a direct phone referral pathway for Nursing and Care homes within the area for which DPT was short listed for National Positive Practice in OPMH and Dementia Awards and received Highly Commended.
