We are looking for highly motivated passionate nurses, to join the Community Mental Health Team Older People, who focuses on providing support and high quality nursing care for your clients and carers, targeting and prioritising people with severe mental health problems. You will manage a caseload of clients, and use evidence based/client centred principles to assess, plan, implement and evaluate interventions as part of the holistic approach. Excellent communication skills are essential, working alongside the MDT, working closely and collaborating effectively with all agencies and partner organisations involved in the care of our clients
The candidate must have a full and valid driving licence and access to a vehicle for work purposes due to the community element of this role.
