Improving outcomes for people with complex care - individual service user funds
Entry Description
Sefton Council in partnership with South Sefton and Southport & Formby Clinical Commissioning Groups, jointly developed and implemented an Individual Service Fund (ISF) model, which was bold and innovative in its approach. We consulted and engaged service users, support providers and advocacy to fulfil our ambition, which was to enable service users to have full choice and control in designing and co-producing their own support service, which met their individual assessed care and support needs, whilst achieving identified outcomes in conjunction with their chosen Individual Service Fund provider.

This award is open to a single council or where appropriate a partnership of councils, or a council-owned company. Private sector partners can enter on a council’s behalf, with the permission of the council itself. With financial pressures and a growing population of older people, and people with disabilities, adult social care is under pressure. This award recognises how councils have innovated to improve or maintain services amid these pressures. It seeks to recognise projects likely to facilitate integration between health and social care, boost personalisation, and improve collaboration between the public, private and voluntary sectors to improve delivery. This award is intended to recognise and promote best practice in this critically important area.
