3. How can pharmacy and medicines’ teams take a leading role to close the gap on health inequalities in their communities?

  • As systems and roles evolve and settle, how can you best manage the impact of change?
  • What should we be doing based on the experience of medicine optimisation teams so far?
  • What role can pharmacy take? Driving seat, shot gun or back seat?
  • Do we create a new initiative requiring a business case and investment or is it better to piggyback on existing initiatives, or simply scale up the work already being done?
  • How should we be utilising and supporting PCN pharmacist resources (and other MDTs) to help close the gap?
  • Empowering teams to leverage support from PCNs/ICBs

Through debate and discussion, you will leave with actions you can implement straight away within your ICS
